Thank you for your interest in attending Provident Living Academy. Please print and complete the Admissions Application and Authorization for Release of Educational Information. Please mail these to the "Spencerport Campus". After receipt of the application, a PLA administrator will contact you to schedule a mandatory entrance interview for both student and parent. Upon completion of the entrance interview, you will be notified of acceptance to Provident Living Academy within 5 days.
Student fees will be due by the second week of the school year, or two weeks after acceptance for mid-year transfer students ($75.00 for each remaining quarter).
Mentoring Plans will be established for each student shortly after acceptance and students will review their plan with a mentor quarterly to adapt and adjust it as needed.
